Certified Med Tech

Autumn House West is a senior living facility seeking Certified Med Techs to join their team. The role involves assisting in the care of residents, administering medications, and supporting daily living activities in a safe and quality environment.

Responsibilities

Prepare and administer each resident’s medication per resident’s service plan and licensed healthcare professional orders
Provide any prescribed treatment to residents
Document all medication passes, and treatments provided
Communicate with Pharmacy for medication changes, refills, missing medications, etc
Communicate with all outside agencies as needed and when they visit the home
Provide support in all areas of daily living as needed by individuals, this may include lifting individuals, assisting in transferring, or moving wheelchairs
Support individuals in obtaining the best quality of life possible
Where assistance is requested/required to support individuals in making choices, advocate for him/her in areas of needs and wants
Assist individuals in developing new relationships and supporting existing relationships
Interacting and communicating in a manner that is respectful toward all individuals
Creating an environment and conducting activities in a manner that ensures health and safety of individuals
Developing and using effective communication with everyone using a style suited to the individual
Always communicating in a positive and productive manner with co-workers, families/guardians and the public
Providing a positive role model for individuals and co-workers
Supporting co-workers and actively participating in team process
Providing complete, accurate and timely documentation
Performing tasks that contribute to the day-to-day operation of the home (such as cooking, cleaning, laundry)
Performing tasks in accordance with Cardinal Policies and Procedures while following all applicable state regulations
Report incidents and accidents as per Cardinal policies and procedures
Notify licensed healthcare professionals of issues or errors regarding medications
Perform narc counts based on Cardinal policy and procedures
Maintaining the home in a safe manner. Notifies someone, i.e., supervisor of any safety or maintenance issues
Perform other duties as directed by the Executive Director, Director of Wellness, and House Manager

Required

High School Diploma or equivalent
Med Techs must be a minimum of 18 years of age
Successful completion of the DHS Medication Assistance Training program
Must maintain a valid and current Medication Administration Certificate and complete annual trainings and certifications as required
Successful completion of the PA DPW Direct Care Staff Person Training Course/Test
Ability to complete the necessary physical movements to move residents by wheelchair
Ability to read, write, speak, and understand the English language
Ability to walk continuously, bend frequently, and sit occasionally
Ability to lift to fifty (50) pounds occasionally (1-33% of workday)
Ability to complete and maintain documentation regarding a resident's status

Preferred

Experience providing basic personal care in a long-term care or healthcare facility
