Overlake Medical Center & Clinics is searching for a proven nursing leader to continue the growth of their Care Transitions Service Line. The role focuses on ensuring the right care is provided in the right place at the right time, overseeing Social Work, Case Management, and Utilization Review functions.

Responsibilities

Ensure that we are providing the right care in the right place at the right time...every time

Report directly to the Chief Nursing Officer with a dotted line relationship to the CFO

Utilize proven change management and process improvement skills plus a strong understanding of hospital operations

Qualification

Healthcare management experienceCase management experienceDischarge planning experienceUtilization review experienceRN licenseAmerican Case Management certificationChange management skillsProcess improvement skillsHospital operationsBachelor's degreeMaster's degree preferred

Required

At least 5 years of healthcare management experience in an acute care setting

At least 3 years of recent case management, discharge planning or utilization review experience

Bachelor's degree

Current RN license in Washington state or a compact state

Preferred

Master's degree preferred

American Case Management Association certification (or equivalent) is preferred
