Responsibilities
Monitor and assess the ongoing health status of residents, communicating changes promptly
Administer medications both orally and via G-Tube, in accordance with physician orders
Provide G-Tube feedings and breathing treatments as prescribed
Support personal care needs, including hygiene and grooming
Foster social interaction and engagement among residents to promote a positive quality of life
Document health status updates, medications administered, and treatments provided in a timely and accurate manner
Collaborate with the interdisciplinary care team to ensure comprehensive, person-centered care
Create a safe, welcoming, and supportive environment for all residents
Participate in ongoing training and professional development opportunities
Other duties as assigned to support resident health and well-being
Qualification
Required
Licensed Vocational Nurse (LVN) in the state of California
Strong communication skills and a compassionate, team-oriented attitude
Ability to administer medications and provide personal care with dignity and respect
Ability to pass a criminal background check, drug screening, health screening, and TB test
