Responsibilities
Provides care coordination and support services to Pathways-enrolled youth and their families
Manages a caseload of clients as defined by the Pathways to Success Program
Maintains weekly contact with clients/families
Develops child and family teams for clients enrolled in the program and facilitates regular team meetings
Provides homebased and community-based services to engage at risk clients and support client self-management of their illness
Develops and maintains client treatment plans
Participates in all required HFS planning, training, and reporting activities
Works collaboratively with community providers
Use of an Electronic Client Record
Demonstrates independent judgment and discretion in making position related decisions
Participates in emergency response activities as assigned
Qualification
Required
Completion of a bachelor's degree in Psychology, Social Work, or a related field and one year of behavioral health experience; or an equivalent combination of training and experience
A valid Illinois Driver's License is required as this position will work within the community
Preferred
Spanish speaking candidate highly preferred
