Responsibilities
Conduct ongoing, holistic assessments of patients and families
Develop and manage comprehensive care plans in coordination with the care team
Facilitate interdisciplinary case conferences and care planning discussions
Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
Advocate for patients and families, addressing physical, emotional, and psychosocial needs
Qualification
Required
Active Massachusetts RN license in good standing
Associate's degree in Nursing required
Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
Strong interpersonal skills and the ability to thrive in a collaborative team environment
Flexibility, self-direction, and sound clinical judgment
Valid driver's license and access to an insured, reliable vehicle
Preferred
BSN or equivalent preferred
