Responsibilities
Provide telehealth services: provide high-quality, evidence-based care to older adults via telehealth
Clinical intakes & evaluations: Telehealth H&P, risk/capacity, assess comorbidities, medication and diagnostic assessments tailored to Medicare patients
Medication management & consults: Prescribe, adjust, and collaborate with therapists; offer asynchronous chart reviews/second opinions
Care team collaboration: Join virtual case rounds with RN Advocates and care coordinators; address SDOH barriers and help de-escalate emerging issues
Geriatric-informed practice: Use geriatric assessment tools to inform treatment plans and referrals
Documentation: Complete timely, high-quality notes—supported by our AI-powered scribe and streamlined workflows
Quality & integration: Contribute to Behavioral Health Integration (BHI) and help refine protocols that improve outcomes and reduce avoidable ER use
Qualification
Required
MD or DO; residency in Family Medicine, Internal Medicine, Psychiatry, Geriatrics, or Palliative Care
Active IMLC Compact license (in good standing); TX and/or FL licenses highly valued; open to adding additional IMLC states
Ability to support the Sailor Health team on a contract, part-time, or full-time basis (at least 15 hours per week)
Comfortable practicing via telehealth and collaborating across fully remote teams
Tech-savvy; quick to learn EHRs and communication tools
Clear, empathetic communicator with a track record of tenure and follow-through in prior roles
Preferred
3+ years post-residency preferred; strong experience with older adults is a plus
