Responsibilities
Provide symptom management and case management for individuals experiencing serious behavioral health disorders
Assist with case management and goal work that is individualized to each person’s needs 6 times per month
Travel frequently within the service area to meet clients in their homes and communities
Qualification
Required
High School Diploma or GED
Experience with clinical documentation, including progress notes, service plans, and discharge summaries
Computer literacy: Proficiency in using a computer for documentation, email, and electronic health records (EHR) systems
Ability to travel frequently within the service area to meet clients in their homes and communities
Strong ability to manage a caseload, prioritize tasks, and maintain organized client files
Basic skills in de-escalation techniques and knowledge of procedures for handling behavioral or psychiatric crises
Exceptional ability to establish rapport and maintain therapeutic relationships with diverse populations
Excellent verbal and written communication skills
Preferred
Associate's or Bachelor's Degree in a related field such as psychology, social work, counseling, or human services
