Responsibilities

Provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management, improving quality of life, and offering emotional and spiritual support
Perform in-depth assessments of patients with serious or life-limiting conditions, develop personalized care plans, and manage their medical needs
Work closely with patients and their families to establish goals of care and ensure treatment aligns with their wishes, emphasizing patient autonomy and dignity
Work in collaboration with a multidisciplinary team (nurses, social workers and other healthcare professionals) to ensure a holistic approach to patient care
Participating in a shared on-call rotation, as directed by the Senior Medical Director

Required

For Physicians: You have an MD or DO degree and are Board Certified or Board eligible in Palliative Care, Hospice & Palliative Medicine, or a related field
You are (or are willing to become) licensed in New Jersey or Georgia, with DEA and CDS registrations
You have a minimum of 3 years of clinical experience in palliative care, hospice, or a related field
For Nurse Practitioners: You possess a BSN from an accredited program and a Master's degree in advanced clinical practice nursing or a post-Master's certificate
You hold an active Nurse Practitioner license to practice in New Jersey, an active DEA registration, and an active CDS registration
You have prior experience working as part of an in-patient palliative care team and/or as a hospice RN
You are based in the state of New Jersey or Georgia

Preferred

Experience in a home-based care setting and/or value based care
Board certified in Palliative Care or Hospice Nursing
