Responsibilities
Conduct comprehensive health assessments, including physical examinations, ordering diagnostics, and developing individualized treatment plans
Emphasize disease prevention, health maintenance, and holistic patient wellness across all age groups
Work in a team-based care model with nurse practitioners, physician assistants, and allied health professionals, ensuring cohesive and integrated care
Engage in outreach, patient education, and preventive care initiatives to reduce health disparities within the community
Accurately document all patient interactions, diagnoses, treatments, and follow-up care in an electronic health record (EHR) system
Participate in quality improvement initiatives, peer review, and evidence-based practice updates to continuously elevate patient care standards
Qualification
Required
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical school
Active Illinois medical license
Board certification or eligibility (to be completed within two years)
Documentation of completed residency
Proven ability to manage both common and complex health conditions within a community health setting
Experience with underserved populations is highly desirable
Strong analytical, problem-solving, and interpersonal skills
Proficiency with electronic medical records (EMR) systems
Ability to work effectively within a multidisciplinary team
