Responsibilities

Maintain the MDS OBRA and PPS schedules
Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of care area triggers and resident care plan
Participate in facility surveys (inspections) made by authorized government agencies
Be highly involved in determining skilled level of care for MCR residents and procuring MCR specific documentation; be responsible for physician certifications of a skilled level of care throughout the MCR stay; be involved in making eligibility determinations
Provide insight and analysis of MDS based Quality Measures
Serve as a member of the quality assurance / QAPI committees
Work closely with the hospital discharge planners, physicians and Admissions personnel to obtain accurate and complete documentation to support ICD-10 /CMI diagnosis coding, IV medication administration and surgical procedures
Participate in the planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility
Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification
Ensure that a comprehensive care plan is completed within twenty-one days of the resident's admission
Ensure that quarterly and annual resident assessments and care plan reviews are made on a timely basis
Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition
Ensure MDS assessments are transmitted timely to the appropriate agencies. Review final validation reports and corrections / modifications in response to warnings / errors as needed0
Attend departmental and staff meetings as directed or called
Attend and participate in in-service educational classes and on-the-job training programs, etc., as scheduled or directed
Perform other related assignments as required
Utilize manual and automated information systems to improve efficiency and effect productivity. Accurately records patient information in the electronic medical records system and patient medical record. Documentation must always be in compliance
Demonstrate a current working knowledge of equipment, instrumentation and medications
Adhere to the standards, policies, procedures and goals of the medical center
Participate in preparation and facilitation of regulatory agency site visits to include DOH
In accordance with agency protocol and DOH standards, maintains adequate infection control standards
Participate in weekly and on an “as needed basis” case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns

Required

Must be an RN and possess, at a minimum, a Nursing Degree from an accredited college or university
Must have, at a minimum, 1 year(s) of experience in a hospital, long-term care facility, or other related health care facility
Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state
Must possess the ability to make independent decisions when circumstances warrant such action
Must possess the ability to interact professionally with personnel, residents, family members, visitors, government agencies/personnel, and the general public
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
Must possess leadership and supervisory ability and the willingness to work effectively with and supervise other personnel
Must be willing to seek out new methods and principles and incorporate them into existing nursing practices
Must be able to relate information concerning a resident's condition
Must be able to cope with the mental and emotional stress of the position
Must possess at a minimum basic computer skills
Must possess in-depth and comprehensive physical assessment skills along with the ability to assess each resident holistically

Preferred

MDS experience or certification is preferred
