ASSISTANT CASE MANAGER

The Assistant Case Manager supports case management for a Community Resource Center, assisting participants and Case Managers with documentation and coordination, while utilizing evidence-based practices.

Responsibilities
The Assistant Case Manager greets participants entering the facility. May complete drug screens according to assigned random schedule for each participant check-in
May perform breath analysis on each participant who enters the facility for any purpose and provides a written or computerized result
The Assistant Case Manager completes initial intake process with participants to completion, including intake paperwork, input into Company database and assignment of groups and other services
Provides general office administration duties including answering phones
The Assistant Case Manager provides general customer service to all who enter the facility, including participants, customers and the general public
Assists Case Managers with case coordination, communication with appropriate agencies and preparation and distribution of reports
The Assistant Case Manager assists in managing a participant caseload using the Principles of Effective Intervention (What Works). Conducts risk/need assessments and creates individual treatment plans for each assigned participant. Meets regularly with participants and documents participants' progress or regress in Company database
Facilitates evidence-based CBT programs and other life skills programs to participants in groups each week. Documents participants' attendance, participation and progress in case management software
The Assistant Case Manager develops and monitors the participant's progress relative to their Case Management Plan through all phrases of the program. Files, prepares charts and performs data entry as assigned
May be responsible for office opening and closing procedures
The Assistant Case Manager may transport participants to the center, community service events, job fairs, employment centers, clinics, using a Company vehicle
May be assigned a small case load and perform the duties of a Case Manager
May require coverage as needed at Porterville location at the Program Manager and Supervising Case Managers discretion
Performs other duties as assigned
Qualification
Required
High school diploma or equivalent required
Must be twenty-one (21) years of age with at least five (5) years driving experience if required to transport participants
Valid State driver's license and a five (5) year record of safe and competent driving as demonstrated by state motor vehicle report if required to transport participants
Ability to safely operate a motor vehicle if required to transport participants
Good customer service skills
Effective communication skills with internal and external contacts at all levels
Good organizational skills and attention to detail
Basic computer skills and proficiency with MS Word and Excel
Basic database skills
Basic math skills
Preferred
BA Degree in Social Services or equivalent
Bi-lingual (English/Spanish) skills
College coursework in Human Services or business discipline
Minimum of six (6) months experience in Human Services field with direct participant services
Minimum of six (6) months experience working with minority populations
