Behavioral Health Case Manager

Responsibilities
Serves as expert behavioral health case manager and member of multidisciplinary team of medical and professionals
Develops, implements, communicates, and coordinates individualized empirically informed patient treatment plans
Chairs and coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting
Develops and recommend strategies to improve healthcare access
Monitors, tracks and intervenes as patients transition between levels of care
Qualification
Required
Bachelor's or graduate/higher level degree: major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position, such as healthcare administration, nursing, social work, psychology, etc
One year of specialized experience which includes performing behavior health assessments; providing individual and group support; and assisting patients with access to healthcare systems and resources
Must possess a valid operator's license in order to drive a motor vehicle to and from work sites
Must obtain and maintain a Basic Life Support (BLS) certification from, or in accordance with, the American Heart Association and American Red Cross
Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact
This position has a mandatory seasonal influenza vaccination requirement
All applicants tentatively selected for this position will be required to submit a urinalysis to screen for illegal drug use prior to appointment
This position is subject to a pre-employment screening, and random drug testing, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident
