Clinical Pharmacy Specialist (Primary Care)

Responsibilities
Provides comprehensive medication therapy management (MTM) through direct patient care to achieve optimal therapeutic outcomes
Prescribes medications and adjusts drug regimens within defined scope of practice
Monitors patients' clinical responses and lab values to ensure safety, efficacy, and adherence
Collaborates with primary care providers and interdisciplinary teams to coordinate and optimize treatment plans
Educates patients on disease states, medication use, and lifestyle modifications to improve adherence and outcomes
Identifies, prevents, and resolves medication-related problems
Utilizes evidence-based guidelines to promote rational and cost-effective prescribing
Provides drug information and therapeutic recommendations to providers
Documents care activities, interventions, and outcomes in the electronic health record
Participates in formulary management and ensures compliance with VA, VISN, and facility policies
Uses telehealth technologies (e.g., VA Video Connect) to provide remote patient care when appropriate
Supports quality improvement initiatives and performance metrics for the Pharmacy and Primary Care services
Serves as a resource for clinical education, mentoring, and staff development within the healthcare team
Qualification
Required
U.S. Citizenship
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Proficient in spoken and written English
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
1 year of experience equivalent to the next lower grade level (GS-12)
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Preferred
ASHP accredited PGY1 Pharmacy Residency
ASHP accredited PGY2 Pharmacy Residency in Ambulatory Care or PGY1 + 3 years > 50% clinical experience in PACT (direct patient care with scope of practice)
BCACP or BCPS certification
Clinical Pharmacy Specialist (PACT) - direct patient care with scope of practice
