Physical Therapist (Home Based Primary Care)
USAJOBS is seeking a Physical Therapist for the Veterans Health Administration. The role involves providing assessment and treatment intervention on an independent basis across the continuum of care and services for patients referred to the Home Based Primary Care program.

Responsibilities
Responsible for the examination and treatment of patients referred to the HBPC
Establishes and maintains contact with other key staff involved in patient care via written, telephone, and personal communication on a regular basis
Conducts comprehensive patient examinations to identify rehabilitation problems and establish treatment goals
Communicates effectively with other internal and external health care providers
Provide education to patients, family members, caregivers, employees, students, and coworkers
Scheduled appointments for assigned patients
Awareness of safety hazards and takes appropriate corrective action
Adheres to infection control programs
Provides training to clinical staff on new techniques and equipment that will assist in the evaluation of the home bound patient
Schedules and completes comprehensive evaluations on patients referred through the program
Work independently in the home solving complex problems relating to Physical Therapy, adapting and modifying the treatment plans, activities, and procedures to meet the needs of the patient with varying degrees of disabilities
Other duties as assigned

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education and Experience: The individual must meet at least one of the requirements below: Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. Doctorate degree in physical therapy
Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university
Foreign Graduates: Graduates of foreign physical therapy programs meet the requirements if they have a full unrestricted and current license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia
Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia
Preferred
Bachelor's degree in Physical Therapy and five years of progressively independent experience as a physical therapist
Master's degree in Physical Therapy and two years of progressively independent experience as a physical therapist
Doctorate degree in Physical Therapy
