PATH is a mission-driven organization focused on assisting individuals experiencing homelessness. The MSW Clinical Case Manager will provide direct client outreach and case management services, aiming to promote recovery and community integration for clients.

Responsibilities

Perform initial comprehensive biopsychosocial assessment; develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services

Collaborate with the client to develop an individualized treatment plan that addresses short- and long-term goals

Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted

Provide field-based therapeutic supportive case management services addressing mental health, functional, and substance use barriers as needed to help clients obtain and maintain housing

Consult with LCSW and supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions, complete critical incident reporting as required per contract and PATH policies

Required

Master's degree in social work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences (e.g., MSW, MFT)

One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations

Have employment eligibility verification

Have or be able and willing to obtain CPR/First Aid training

Successfully complete the following as a condition of hire: Tuberculosis Test, Background Screening, Drug Test

Have reliable transportation

A valid driver's license

Proof of insurance and ownership for personal vehicles used during work duties

The ability to qualify for PATH's insurance coverage
