Responsibilities
Support the Case Managers, Medical Social Workers, Nursing staff, and other members of the health care team in meeting discharge planning and continuity of care needs for patients
Participate in orientation and continuing education
Update and maintain knowledge and skills related to specific areas of expertise
Complete population specific competency annually on populations served as identified in scope of care and service
Qualification
Required
Associates degree in Business or Health related Field
3 years of relevant experience (preferably in a health related setting) or equivalent combination of education and experience
excellent written/oral communication skills
organizational and interpersonal skills
Working knowledge of computers
PC and appropriate software knowledge
Effective communication skills both oral and written
good interpersonal, motivational and strong organizational skills including time management
Customer service orientation
