Medication Historian Tech (Rotating Shift)

The role involves collecting, clarifying, and documenting accurate medication histories under the direction of a pharmacist, while also mentoring new technicians and ensuring proper documentation in a computerized system.

Responsibilities
Prioritize workload based on patient’s risk level and admissions
Interview patients / family members/ caregiver to obtain a complete and accurate medication history including drug allergies and reactions
Compile a complete and accurate list of targeted patient’s home medications including over the counter drugs and herbal supplements including drug name, dosage, frequency, compliance and last dose taken
Use available resources to reconcile the patient’s medication list including physicians, pharmacies, nursing homes, payers, etc
Notify healthcare provider of any omission, duplications, errors, or other important details related to the patients home medication list
Document findings by accurately entering all available medication-related data into the computer system
Mentor and assist with training new Medication Historian Technicians
Collect and enter data into the pharmacy information system as directed
Monitor outcomes of medication reconciliation process and reporting to appropriate committees
Attend mandatory staff meetings
Perform all duties of a pharmacy technician II if appropriately trained

Perform other job related duties and projects as assigned
Collect data, conduct quality monitors and complete documentation as assigned
Maintain logs, records and other required documentation accurately, files documents in appropriate locations
Qualification
Required
Requires high school diploma or equivalent
A minimum of one year of pharmacy technician related experience and/or equivalent medication historian related experience required
Current State of Michigan Pharmacy Technician license or equivalent licensed healthcare professional
Interpersonal skills to effectively interact with patients and all levels of hospital and medical staff
Thorough knowledge of pharmaceutical products, generic names, routes of administration, dosage forms etc
Working knowledge of medical abbreviations, medical terminology and disease states
Good level of judgment and ability to work independently
Typing skills (30 words per minute)
Ability to learn new computer programs in order to quickly and efficiently navigate clinical information systems and pharmacy-related databases
Excellent organizational skills required due to the fast-pace and dynamic workflow for this role
Ability to pay close attention to detail
Demonstrate basic mathematical skills
Successfully complete competency exam with a minimum passing score of 80%
Demonstrate knowledge of medications via skills review
Presents and operates in a professional manner which commands respect, reflects a high level of competency, and encourages congenitally
Preferred
Prior outpatient/retail pharmacy technician experience is preferred
