Director of Clinical Integration
Bowen Health is a state-of-the-art healthcare organization committed to compassionate care. The Director of Clinical Integration will collaborate with various leaders to ensure smooth transitions between clinical services and innovate processes to meet patient needs.
Responsibilities
Develop and expand access points as well as transition workflow and systems to support access to and from each care setting across service continuum and allow for simultaneous care in different care settings
Develop systems and standards for collaborative care and information sharing with external care providers and referral sources, both sending and receiving referrals and collaborating care decisions with a care model of Bowen Health as patient’s total healthcare home
Develop systems for information sharing and care collaboration with key operational partners facilitating care provided to patients at Bowen Health such as pharmacy, lab and diagnostics
Create electronic medical record workflow alignment to support care collaboration and integration
Create system that allows ease of data collection and tracking of key performance indicators around clinical care delivery
Create, update and align policies with evolving care standards and service delivery
Foster a culture of compassion, connection, and accountability in care delivery, with patient centered care at forefront of care delivery model
Seek continuous improvement utilizing process improvement strategies and interdisciplinary review
Lead change across disciplines
Qualification
Required
Licensure or certification in a clinical discipline (e.g., RN, NP, etc.) is highly desirable
Proven success in clinical operations management
Proven success in integrated care delivery across multiple service lines (mental health, substance use, primary care, etc.)
Proven success in process improvement and change management
Proven success in EMR optimization and data-driven decision making
Experience working with interdisciplinary teams and external partners (labs, pharmacies, referral networks)
Excellent collaborator and communicator across clinical and administrative teams to drive integration
Utilizing input (surveys, panels) from patients, external partners, and community stakeholders to guide continuous improvements
Ability to update and align policies with evolving care standards
Comfortable navigating change and uncertainty
Seek to explore new systems and technologies, develop plan, and implement
Committed to ethical care delivery
Value transparency in documentation and patient access
Strong writing and documentation skills
Skilled in data driven decision making
Strong background in process improvement
Preferred
Master's degree in a healthcare-related field is strongly preferred. Examples: Healthcare Administration, Public Health, Nursing, Social Work, Clinical Psychology, or an MBA with a healthcare focus
7–10 years in healthcare settings, with 3-5 years in integrated care system and leadership/administrative role preferred
