Responsibilities
Ensure that Initial Nutritional Assessments on all admissions are conducted within 14 days of admission. Complete quarterly & annual assessments per SCHC's schedule. See that Nutrition section of MDS, RAPS & Resident Care Plan is done in timely manner per SCHC’s schedule
Develop and update nutrition care plans, as appropriate, and monitor follow through by staff
Work with DON & nursing staff to identify residents at nutritional risk for tube feedings, weight loss and pressure ulcers. Assess, make recommendations and monitor progress. Participate in care conferences for residents at nutritional risk, as may be appropriate and follow
Counsel residents & families, as appropriate, concerning nutritional problems or needs
Assures that all residents have a Physician's Diet Order and that said diet orders are followed and function smoothly
Periodically review systems for monitoring residents' food & fluid intakes for meals & snacks. Make recommendations, as appropriate
Periodically monitor meal service in conjunction with nursing & dietary staff to ensure diets are served as written. Provide training to dietary and nursing staff as indicated when diets are not served as proscribed
Review Dietary Department sanitation and sanitary practices with FSD, as appropriate. Identify problems and make appropriate recommendations to FSD
Review the written plans for Dietary in-service training schedule; Conduct those in-services identified within RD's area of expertise. Consult FSD to make recommendations, as may be appropriate
Review & approve all regular and therapeutic menus at start of each menu cycle
Participate in appropriate sessions of SCHC's in-service education programs. Participate as a member or advisor in committees or review groups of SCHC, as appropriate
Provide professional guidance and consultation, as may be appropriate, directly to supervisory personnel responsible for the function of SCHC, which relates to RD's profession
Qualification
Required
Credentialed with the DCR(Commission for Dietetic Registration) of the American Dietetic Association (is a Registered Dietitian)
Licensed as a Registered Dietitian within the State of New Hampshire
Experienced in Geriatric nutrition and clinical dietetics
Engages in continuous self-development to improve knowledge and skills in Dietetics
Possesses a high standard of ethics
Has ability to educate and train
Has knowledge of federal, state, and local regulations
Works well with people to provide services in a non-discriminatory manner without distinction or regard to race, age, sex, color, creed, religion, national origin or handicap
