Client Care Advisor

Responsibilities

Ensure documentation and care plans meet all compliance requirements
Support quality improvement and staff training
In Home Visits to enhance care, experience, and retention
Promote a culture of safety and excellence
Oversee and maintain the quality, accuracy, and compliance of all client care documentation and service delivery
Develop, review, and update care plans to reflect clients’ evolving needs
Provide ongoing education and support to caregivers and families to improve understanding and care outcomes
Monitor client satisfaction, engagement, and retention, ensuring issues are addressed quickly and compassionately
Conduct quality assurance audits, identify trends, and report findings to leadership
Support continuous improvement initiatives that enhance the client experience and caregiver performance
Partner with the team to keep clients safely on service and at home
Conduct regular chart and documentation audits for accuracy, completeness, and regulatory compliance
Review assessments, care plans, visit notes, and discharge summaries for quality assurance
Monitor quality metrics, patient satisfaction, and incident trends; prepare and present QA/QI reports to leadership
Coordinate and participate in Quality Improvement (QI) activities to enhance care outcomes and service delivery
Provide feedback, education, and training to staff on documentation, compliance, and quality standards
Ensure care is accurate, provide activities
Help monitor the care our clients receive in the home - support families with resources
Support the development and implementation of policies and procedures to improve overall quality and compliance

Required

Education: Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred; equivalent experience in healthcare quality assurance accepted
Experience: Minimum 2-3 years in home health, home care, hospice, or healthcare quality/compliance
Knowledge of: State and federal home care regulations, OASIS documentation, HIPAA, infection control, and accreditation standards (e.g., Joint Commission, CHAP)
Skills: Strong attention to detail and analytical ability
Skills: Excellent written and verbal communication skills
Skills: Proficiency in EMR systems (Homecare Homebase, AlayaCare, MatrixCare, or similar)
Skills: Ability to work independently and collaboratively across teams
