Quality Data Coordinator

Responsibilities
Effectively and efficiently provide clinical data for the purpose of driving performance improvement activities to ensure positive patient outcomes
Coordinate with the Quality Performance Improvement Specialist and other operational team members to facilitate performance improvement teams within a service line model
Work with the analyst to produce and submit clinical data reports as requested by administration, clinical departments or committees using available information systems and databases
Participate in statistical analysis, interpretation and monitoring of clinical data
Understand and perform data abstraction requirements
Conduct chart reviews
Review data for external data requirements and benchmarking
Attend and support applicable/ assigned committees and PI Teams
Perform chart review and data abstraction duties from the medical record
Provide for accurate and timely collection of clinical data for the purpose of driving performance improvement activities in the hospital to ensure excellent patient care outcomes
Support use of evidence based quality indicators by providing current real-time clinical data for clinicians and administration to direct patient care; address PI outcomes and operational goals of the organization
Qualification
Required
Degree in Nursing, Bachelor's degree preferred with minimum 3-5 years of hospital nursing experience
Undergraduate clinical degree or computer science information degree required with 5+ years healthcare related work experience required
Active RN licensure required, if applicable
Must be proficient with Word, Excel, and other Microsoft applications
Knowledge base of clinical core measures and abstraction process
Must possess excellent verbal and written communication skills
Must have excellent interpersonal skills
Must be self-directed and highly motivated with problem-solving abilities
Individual must be self-confident and self-motivated and able to function in high stress environment subject to interruption and changing priorities
Must possess excellent verbal and written communication skills and be able to interact collaboratively with various internal and external hospital contacts
Preferred
At least 1-3 years of performance improvement and metric abstraction experience preferred
Prior experience in chart review and data abstraction strongly preferred
Knowledge of The Joint Commission (TJC) and CMS terminology, and of ICD-9/ DRGs preferred
Knowledge of current Performance Improvement methodologies and performance measures (i.e. FOCUS PDCA, Lean, Microsystems, chart and graph interpretation) preferred
