Responsibilities

Direct patient care delivered in both community pharmacy settings

Public health activities: community outreach programs

Development of collaborative program to improve adherence and/or outcomes

Perform medication therapy management

Deliver disease state management programs

Recommend and administer vaccines

Develop or enhance existing patient care programs

Complete a teaching certificate program

Provide clinical instruction and serve as a preceptor to student pharmacists

Design and conduct a practice based research project

Present research project findings at appropriate regional and national professional conferences

Qualification

Doctor of Pharmacy degreePharmacist licenseCertified ImmunizerMedication therapy managementPatient care programsTeaching certificatePublic health activitiesResearch project

Required

Doctor of Pharmacy degree from a college or school of pharmacy accredited by the Accreditation Council for Pharmacy Education

Ability to obtain pharmacist license within the state of residency within the licensure requirement deadline

Submission of residency application materials to Pharmacy Online Residency Centralized Application Service (PhORCAS)

Individuals must participate in and adhere to the rules of the ASHP Resident Matching Program

Certified Immunizer or willing to become an immunizer within 90 days of hire
