Clinical Pharmacy Specialist (Oncology)

Responsibilities

Provides comprehensive medication therapy management for oncology and hematology patients
Reviews, verifies, and adjusts chemotherapy and biologic regimens for indication, dose, and toxicity
Monitors therapeutic response, adverse effects, drug interactions, and organ function
Counsels patients and families on chemotherapy, biologics, and side-effect management
Manages infusion-related and hypersensitivity reactions in clinic settings
Provides pharmacokinetic consultations and therapeutic drug monitoring
Assists in supportive care management including pain, nausea, hypercalcemia, and infection
Conducts telehealth assessments and manages treatment remotely
Evaluates and approves non-formulary and restricted medication requests
Provides drug information and recommendations to multidisciplinary care teams
Coordinates with oncology, hematology, and infusion services to optimize therapy
Precepts and trains pharmacy residents, students, and new staff
Educates nursing, pharmacy, and physician staff on chemotherapy and cancer pharmacotherapy
Designs, supervises, and supports post-doctoral oncology residency training
Participates in and publishes oncology-related clinical research and case studies
Reviews oncology order sets and formulary content for accuracy and policy compliance
Prepares drug utilization evaluations and quality assurance reports
Serves as liaison between oncology pharmacy and hospital departments
Represents Pharmacy Service in institutional committees and initiatives
Maintains licensure, certifications, and continuing education
Ensures adherence to safety procedures and proper use of protective equipment
Complies with patient privacy, computer security, and documentation standards

Required

U.S. Citizenship
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Proficient in spoken and written English
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
Must complete all application requirements detailed in the 'Required Documents' section of this announcement
1 year of experience equivalent to the next lower grade level (GS-12)
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters

Preferred

ASHP accredited PGY1 Pharmacy Residency
PGY2 Residency in Oncology Pharmacy or PGY1 + 3 years >50% clinical experience in Oncology
BCOP certification
Clinical Pharmacy Specialist (Oncology) - direct patient care with scope of practice
