Responsibilities
Provide services within the scope of Nursing, in accordance with the plan of care, and in coordination with the interdisciplinary Team
Follow plan of care; provide treatment; evaluate patient’s reaction to treatment
Report any changes in his/her condition to the hospice Clinical Director
Online Charting Kinnser in a timely manner on company-provided devices
Be responsible for following all company policies and procedures
Perform other duties as designed
Qualification
Required
One year of previous nursing experience
Current LVN License to practice in California
Current CA Driver's License
Preferred
One year of Hospice or Home Health experience
