Responsibilities

Functions as one of the Medical Center's Case Managers for VA Black Hills HCS, Health Care for Homeless Veterans Program HUD/VASH Program. The HUD/VASH program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services. The primary goal of HUD/VASH is to move Veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve Veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated

Monitors and administers HUD Vouchers for homeless Veterans, serving as the primary contact with local Public Housing Agencies (PHAs) and provides case management-based services (directly or through referral) necessary to ensure that Veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations

Establishes, implements, and maintains referral and screening procedures for potential HUD/VASH Veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs

Conducts and participates in outreach activities including field interviews, assessments and referrals for homeless Veterans

Assesses at-risk factors and develops a preliminary plan, involving the Veteran and family or significant others, and performs an insightful assessment of serious and complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses

Assists Veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with Veteran's goals associated with healthful independent living

Gives advice, guidance, emotional support and other assistance and provides individual and group counseling services as well as crisis management services needed to maintain the Veteran safely in their residence

Establishes a continuing relationship with the Veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate. The incumbent meets Veterans to assess accomplishments and re-establish goals and monitors Veteran's progress, maintains comprehensive documentation, ensures expert diagnosis and treatment of clinical disorders

Must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment

Education - Master's or doctoral degree in Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; OR a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP)

Knowledge, Skills and Abilities for GS-09 LPMHC: Ability to engage in intake and assessment, problem identification, diagnosis, treatment planning and implementation, continuum of care, evaluation and follow-up

Knowledge of current Diagnostic and Statistical Manual (DSM) of Mental Disorders in formulation of treatment goals and application of appropriate clinical interventions using professional counseling practices

Basic knowledge of professional counseling practice with emphasis on human development principles through cognitive, affective, behavioral or systemic interventions, and strategies that address high risk behaviors, wellness, personal growth, and career development across the life span

Ability to use a wide variety of individual, group, or family counseling interventions

Ability to serve on an interdisciplinary team with focus on the intrapersonal, interpersonal, and relational functioning of individuals, groups and families and on the counseling approaches, strategies, and techniques that will best help patients function effectively

Basic skill in the use of computer software applications for drafting documents, data management, maintaining accurate, timely and thorough clinical documentation and delivery of services, such as Telehealth

Ability to learn and utilize software programs in use by VHA

Knowledge and understanding of existing relevant statutes, case laws, ethical codes, and regulations affecting professional practice of counseling

Ability to communicate effectively, both orally and in writing, with people from varied backgrounds

Knowledge, Skills and Abilities for GS-11 LPMHC: Ability to make professional evaluations, decisions, and recommendation for treatment planning and implementation

Advanced knowledge and mastery of the fundamentals of the counseling process which includes defining patient/family problems and maintaining an effective counseling relationship

Ability to provide subject matter consultation to colleagues and trainees on the counseling process within various specialty areas

Ability to provide complex crisis intervention and stabilization to patients who are in psychological distress

Ability to establish goals/treatment through a collaborative process with the patient utilizing advanced counseling skills, including evidenced-based practices, screening, and psychosocial assessment

Ability to develop and facilitate psychotherapy and psycho-education groups that include life skills, family support, and community integration

Benefits

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
