HMR Veterans Services, Inc. is dedicated to serving the Nation's Heroes, and they are seeking an experienced Director of MDS to lead clinical documentation systems across their long-term care communities. This role involves overseeing MDS processes, ensuring compliance, and driving performance initiatives to improve quality measures and facility ratings.

Responsibilities

Provide leadership, training and oversight to facility MDS Coordinators and interdisciplinary teams

Monitor facility performance in MDS accuracy, reimbursement, Quality Measures and 5-star ratings

Conduct audits and data analysis to identify trends, compliance risks, and opportunities for improvement

Collaborate with operations, clinical, and IT leaders to optimize data integrity, workflow efficiency, and system utilization

Provide onsite and remote support to facility teams; up to 50% travel required

Required

5+ years of long-term or post-acute care experience, including leadership or corporate-level oversight

Expertise with EMR systems (PointClickCare preferred) and MDS coordination

Strong knowledge of federal and state long-term care regulations

Excellent communication, training, and project management skills

Preferred

Registered Nurse (RN) license preferred; or LPN with 10+ years MDS experience and at least 2 years multi-site experience
