Responsibilities

Greeting patients in a courteous and sensitive manner
Scheduling appointments and assisting with the needs of clients
Offering patient intake interviews, financial screenings and registering for Ryan White and AIDs Drug Assistance Program (ADAP) services
Ensuring appropriate documentation for patient eligibility is filed and entered into the computer database
Accurately collecting new patient medical history
Determining what tests/screenings are due during medical visits
Taking patient vitals and documenting all patient health-related complaints
Communicating with patients between provider visits for condition updates
Managing the referral process by processing specialty referrals in a timely and accurate manner
Providing all pertinent information to patients for timely appointment scheduling
Assisting with appointment scheduling as patients require
Obtaining specialist consults and scanning into the patient’s EMR
Tracking and monitoring the status and progress of the referral being completed
Notifying a provider of a patient’s lapse in completing the referral
Remaining in contact with the patient to improve adherence
Adding consistent value to the Healthcare Center Team especially in high-stress environments
