Transition Support Team Lead Coordinator (LPC, BCBA, LCSW)

The Transition Support Team Lead Coordinator will be part of a multidisciplinary team providing support services through case consultation, training, and education related to IDD and mental health, aimed at preventing institutionalization and supporting those transitioning from institutional settings.

Responsibilities
Create educational opportunities, such as webinars, conferences, training, and specified correspondence, focused on increasing the expertise of LIDDA and provider staff in supporting the individuals with IDD at risk for institutionalization or those who have moved from institutions (SSLCs, NFs, and medium or large ICF/IIDs). Request pre-approval of new educational opportunities in accordance with LIDDA Performance Contract
Provide technical assistance, upon request from LIDDAs and providers, on specific disorders and diseases, with examples of best practices and evidence-based services for individuals with significant medical, behavioral, and psychiatric challenges. Conduct community outreach to inform agencies of the availability and type of educational opportunities and outline how to request educational opportunities from their region's LIDDA
Facilitate case-specific peer review or multidisciplinary consultations to support service planning teams that need assistance planning and providing effective care and create and provide the suggestions within five business days to the referring person
Maintain compliance with required HHSC and EHN training
Schedule and facilitate a stakeholder committee meeting at least annually
Schedule, facilitate, and attend monthly TST office hours with the other TST leads to discuss challenges, successes, upcoming educational opportunities, share community resources, and collaborate on program initiatives
At least once per quarter, communicate face to face, either in person or through a virtual platform, with the DSA LIDDAs (Concho Valley, Permia Care, and West Texas) and providers to inform them of the availability of all program services and outlines how to request a service
Develop a TST program plan in conjunction with the Director of Care Transition, to be submitted annually to HHSC for review and approval in accordance with LIDDA Performance Contract
Participate in the development of the Calendar Year Contractor Form
Complete the Quarterly TST report in accordance with HHSC requirements and comply with internal/external submission due dates
Complete Annual Satisfaction Surveys in accordance with HHSC requirements
Performs other duties as assigned
Out-of-town Travel not limited to the Designated Service Areas (Concho Valley, PermiaCare, and West Texas) to provide support and/or education/
Local travel to provide training, technical assistance, and outreach activities
Logs and maintains accountability of training and peer reviews on a day-to-day basis
Performs other duties as assigned
Qualification
Required
Master's degree in psychology, Social Sciences, or related behavioral field, supplemented by two (2) years of experience; or possession of any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities
Requires a Licensed health-related professional with direct work experience in the IDD field
Requires at least one of the following: Board Certified Behavior Analyst (BCBA), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Texas Registered Nurse (RN)
Must possess and maintain a valid state Driver's License with an acceptable driving record
Must be able to pass a TB, criminal background, and drug screen
Preferred
Experience with trauma-informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives
Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions)
