Responsibilities

Provide overall direction and clinical management for home care services, ensuring compliance with all state and federal regulations

Take, review, and convert home care referrals; collaborate with office staff to ensure cases are appropriately authorized for services

Conduct and supervise client care, including assessments, recertifications, care plan development, coordination of services, discharge planning, and periodic evaluation of client care

Receive, document, and assist in resolving complaints

Oversee Home Health Aide requirements, including in-service training and competency evaluations

Partner with the Branch Director on the Quality Assurance and Performance Improvement (QAPI) program

Conduct recertification, supervisory, and other home visits as needed

Support office staff with recruitment, new hire processing, and scheduling issues

Collaborate with office staff to implement a recruitment program ensuring staff growth and adequate coverage of existing cases
