Responsibilities
Support Our Clinicians: Provide essential care coordination and logistical support to field staff
Manage Supplies: Oversee patient supply orders, tracking, and delivery
Monitor Patient Care: Help ensure excellent clinical outcomes through vigilant care oversight
Conduct Outreach: Perform triage and wellness calls, assisting with clinical follow-up as needed
Manage Communications: Handle incoming clinical faxes and route them appropriately
Coordinate Scheduling: Collaborate with the Scheduling team to meet patient care needs
Qualification
Required
Active California RN License
Minimum 1 Year Nursing Experience (Home Health experience preferred)
Strong Communication Skills: Professional phone manner and interpersonal skills
Tech Proficiency: Comfortable with EMR systems and Apple devices
Highly Organized: Ability to multitask and prioritize in a high-volume office setting
Team Player: Ready to collaborate across departments to deliver exceptional care
