They are looking for an experienced Admissions RN responsible for conducting initial in-home evaluations and developing care plans, ensuring effective communication with the ordering provider.

Responsibilities
Conducting the initial in-home evaluation of the patient
Development of the care plan under the direction of the MD
Identify a patient’s needs at home and make referrals to other disciplines
Communicate effectively with the ordering provider when conducting the initial assessment to ensure accuracy of the treatment plan and all medications
Qualification
Required
Current Massachusetts Nursing license in good standing
2+ years of nursing experience required
Strong clinical assessment skills and the ability to prioritize and triage patients
Exceptional communication skills and the ability to work within a team structure
Excellent organizational skills and time management skills
Ability to work independently and understanding of how to access resources for patients
Strong computer skills, experience in Oasis
Preferred
Registered Nurse (Bachelor's degree preferred)
