Responsibilities

Serve as the attending physician for residents in each assigned SNF
Collaborate with onsite NPs/PAs to manage daily patient care and oversee clinical supervision
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions
Complete timely documentation in ChartEasy™ and work with facility EHRs (PointClickCare, MatrixCare, AHT)
Participate in Theoria’s Chronic Care Management, Transitional Care, RPM, and other quality initiatives
Utilize electronic medical systems, including EHRs and state/county tools for clinical and administrative tasks

Qualification

MD LicensePrimary Care ExperienceEHR ProficiencyChronic Care ManagementTransitional CarePatient-Centered CareClinical SupervisionCollaboration Skills

Required

MD degree
Current medical license in the state of Kansas
Experience in post-acute care settings
Ability to collaborate with onsite NPs/PAs
Strong documentation skills using EHR systems
Commitment to high-quality, patient-centered care
Ability to diagnose and treat acute and chronic conditions
Familiarity with electronic medical systems, including EHRs and state/county tools
