Silver Birch of Avondale is seeking an outstanding Resident Care Coordinator (RCC) to join our team. The role involves managing resident care, coordinating staff schedules, and ensuring compliance with care standards.

Responsibilities

First point of contact for families, visitors, physicians, and providers

Maintains staff communication from shift-to-shift

Manages medications, including cycle fill

Completes monthly audits

Manages new resident ALIS profiles

Ensures incident reports are completed

Ensures ADLs completed and documented

Prepares resident service plans

Manages staff schedule and covers open shifts, as needed

Coordinates transportation for residents

Trains new Med Techs
