Clinical Pharmacy Specialist - Cardiology

Responsibilities

Promotes a patient-focused, team-based multi-disciplinary model of pharmaceutical care through drug prescribing, medication monitoring, physical assessment and drug laboratory review that assures positive outcomes from medication use
Participates in the provision of pharmaceutical care support services as required including drug distribution, drug information, database management and documentation of interventions
Apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities
Utilizes expert knowledge of clinical pharmacokinetics and pharmacodynamics to make dose adjustment based on objective laboratory findings
Ensures pharmacists understand how to report actual and/or potential medication errors or adverse events and participates in quality improvement initiatives to promote patient safety
Actively participates in the Adverse Drug Reaction (ADR) reporting program
Documents clinical interventions and therapeutic plans accurately in CPRS in a timely and professional manner
Works collaboratively with clinic providers to provide pharmaceutical care to selected patients as outlined by best evidence, VHA and BPVAHCS clinical practice guidelines. Clinical activities include histories, evaluations, outlining and executing, and monitoring pharmacotherapeutic treatment plans utilizing the most effective, least toxic and most economical medication treatments
Assists in the training of clinical clerkship students during four to eight week ambulatory care rotations
Responsible for coordinating and supervising the activities of postgraduate residents in ambulatory care in the capacity as preceptor throughout the academic year
Provides therapeutic lectures to pharmacy students and residents in cooperation with Assistant Chief, Clinical Pharmacy Services, the Disease Management Supervisor and the Special Project Coordinator
Regularly schedules educational programs for the pharmacy staff, patient care area and/or Cardiology providers
Serves as an authoritative information source on medication and their uses. Provides both written and oral drug information to physicians, nurses and other associated healthcare professionals as well as to patients and their caregivers
Responsible for the daily operation status of various aspects of the pharmacy programs
Prioritizes work and makes administrative decisions when supervisory pharmacists are not available, or in stances where immediate decisions are required
Supervises technical support staff during holiday and off tours
Utilizes innovative and creative procedures to implement cost containment
Responsible for the safe and appropriate distribution or dispensing of medications
Ensures that all Pharmacy Service and other regulatory policies are compliant with in regards to medication labeling, inventory management, formulary management, PBM initiative, equipment maintenance and timeliness of service
Provides supervision of Pharmacy support staff in the conduct of distributive functions

Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree or on track to complete advanced degree from an accredited program by the time of hire
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters

Preferred

Completion of PGY2 Pharmacy Residency in Cardiology, or equivalent work history with current scope of practice/prescribing privileges in Cardiology setting
Board Certification from the Board of Pharmacotherapy Specialties in a relevant care of specialty such as (BCPS, BCCP, BCCCP, BCACP)
Experience in transitions of care of cardiology patients, including heart failure
Experience in population health management of cardiovascular patients including heart failure
