Nurse Practitioner- Pediatric (Full-Time, 0.8/Days)
Hospicecommunity is seeking a Nurse Practitioner specializing in Pediatrics to join their Sunflower Pediatric Hospice and Palliative Medicine Interdisciplinary Team. The role involves providing clinical expertise, collaborating with healthcare teams, and addressing the comprehensive needs of patients and families in hospice and palliative care.

Responsibilities
Serves as a member of the Sunflower Pediatric Hospice and Palliative Medicine Interdisciplinary Team (IDT)
Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving hospice and palliative medicine services
Provides clinical direction and expertise to the Sunflower IDT
Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Choices Healthcare standards and guidelines
Collaborates with Choices Healthcare physicians and/or Pediatric Medical Director and the Sunflower IDT in admission, certification, recertification, and discharge decisions
Collaborates and maintains communication with the patients’ healthcare team (e.g. primary care provider, subspecialty teams, and hospice and palliative medicine physicians) as needed for the coordination of hospice care and palliative medicine services
Monitors plans of care to assure consistency with Choices Healthcare philosophy, policies, procedures, and protocols
Provides clinical evaluations for hospice and palliative medicine patients when necessary and requested as part of the plan of care, and documents visits and services according to Medicare and Choices Healthcare standards
Provides support and education to patients, caregivers and facility staff and acts as a liaison to facilitate goals of care
Participates in interdisciplinary team meetings as scheduled
Participates in learner education (e.g. medical student, residents, and fellows) as needed
May be required to participate in some adult hospice responsibilities as directed by supervisor PRN
Qualification
Required
Certified Nurse Practitioner license in the Commonwealth of Pennsylvania as Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP)
Certification in Hospice & Palliative Medicine required within three years from date of hire
1 year of pediatric experience required
Prescriptive authority and unrestricted DEA license required
