As a Transition Navigator, you will be responsible for the recruitment and case management of students enrolled in the Pathways to Progress program, collaborating with stakeholders to coordinate career exploration services.

Responsibilities
Recruiting eligible students for the P2P program
Participating in outreach activities to promote the P2P program
Working collaboratively with stakeholders, assigned school districts, and Centers for Independent Living
Provide creative and innovative opportunities for students to engage in career exploration
Networking with local employers to create positive relationships and opportunities for student work experience sites
Providing basic case management for students enrolled in the P2P program to ensure they have access to partner services and mental health care as applicable
Serving as a mentor to students by offering support, encouragement, and inspiration as they explore their futures
Qualification
Required
Knowledge of Google Suite programs
Knowledge of complex case management
Knowledge of local area schools and businesses
Preferred
An associate's degree in counseling, education, social work, business administration, or a related field is preferred
