Responsibilities
Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process. Maintain a caseload and productivity per agency’s protocol
Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
Maintain regular communication with Director of Patient Services/designee to review caseload
Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions
Qualification
Required
One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing
Current license as a registered professional nurse in the State of California
Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car
Preferred
Experience in acute care/ home health and/or hospice setting preferred
Public Health Nursing certification highly desirable
HPNA certification preferred
