Responsibilities

Assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs

Managing a caseload that includes supportive and medically complex members

Developing a proactive course of action to address issues presented to enhance short and long-term outcomes and opportunities for members' overall wellness

Determining appropriate services and supports based on members' health needs, including Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Required

Must live in Tampa, FL (Specifically in zip code 33614 or within a 2-3 mile radius)

Bi-lingual (English and Spanish)

Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL

1+ years of experience in behavioral health or a long-term care setting

Computer proficiency in Microsoft Word, Excel, and Outlook

Bachelor's Degree required, preferably in Social Work or related field (social services field - psychology, health administration)

Preferred

Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment

Effective communication skills, both verbal and written

Managed care experience

Case management and discharge planning experience
