Responsibilities
Responsible for individualized case planning, discharge planning, and monitoring progress
Responsible for case management duties to include planning and implementing all facets of programming and associated documentation
Prepares, organizes, schedules, and implements psychoeducational groups and interventions
Monitors patient compliance with established goals and objectives; updates case plans as needed
Qualification
Required
Bachelor's Degree (typically in Social Services)
0-2 years of progressive work experience (typically in Casework)
3-5 years of progressive work experience (typically in Casework) if education and experience substitution is applicable
Knowledge of human behavior, motivation, and interpersonal behavior
Knowledge of the varied techniques of programming/education
Knowledge of social work principles and practices
Knowledge of interviewing and counseling techniques
Knowledge of current social, economic, and health problems and community resources
Physical ability to lift, twist, bend, pull, and push up to 50 lbs
Preferred
Currently a certified Social Worker
