Population Health Analyst

Responsibilities

Gathering, cleaning and analyzing population health data from various sources, including electronic health records (i.e. Epic, Athena, eCW), claims data, surveys, and other relevant sources (i.e. Arcadia)
Identify patterns, trends, and correlations in data to assess the health needs and risks of different populations
Support the development of population health strategies and interventions based on data analysis and evidence-based practices
Collaborate with internal and external stakeholders to collect, validate, and analyze data to assess the effectiveness of population health programs
Prepare and present reports, dashboards, and visualizations that communicate key findings and recommendations to stakeholders
Monitor and evaluate the performance and impact of population health initiatives/programs, including tracking key performance indicators and outcomes
Effectively communicating findings and recommendations to diverse audiences, including clinical staff, community partners, and policymakers
Leading and managing analytic projects, including vendor partnerships and the development of analytic tools
Support, train, and guide cross-functional team members on accurate data extraction and analysis, ensuring alignment with performance goals and programmatic strategies to drive informed decision-making and continuous improvement
Lead meetings focused on performance improvement and program development, providing strategic insights, facilitating stakeholder engagement, and driving alignment on goals, initiatives, and next steps
Stay current on population health trends, best practices, and emerging technologies to support continuous improvement and innovation

Required

Bachelor's degree in healthcare management, public health, health informatics, epidemiology, statistics, or a related field
Proven experience in population health analysis and program evaluation, including population management programs
Strong analytical skills and proficiency in data analysis tools and software
Knowledge of statistical methods, epidemiology, public health principles, and health disparities
Strong leadership and management skills
Collaborative and team player
Knowledge and understanding of value based care, risk contracts
Understanding of process improvement tools and techniques
Facility with Microsoft Word, Excel, Power Point

Preferred

Master's degree
Experience with Epic, Epic Healthy Planet and Arcadia
