Responsibilities
Provides care management and care coordination for patients enrolled in the Care Management Program in the Primary Care setting
Collaborate with specialists, members of the health care team and patients/families/caregivers to ensure the delivery of quality, efficient and cost effective health care services
Assesses care plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the patient’s health status
Qualification
Required
Current Hawaii Registered Nurse (RN) license
Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card
Current driver's license and valid Hawaii auto insurance with short notice access to an automobile to conduct site visits, as required
At least two (2) years of clinic experience in a Primary Care setting
Experience working within a collaborative interdisciplinary team
Preferred
Certification in Case Management
Experience in care coordination for patients
Care management experience
Experience as participant in continuous quality improvement
Working knowledge of EMR (Electronic Medical Record)
