RN Quality Improvement Specialist

Responsibilities
Abstraction of quality measures from electronic medical records systems using measure specifications
Auditing clinical documentation based on expected best practices
Participation in improvement projects
Review patient charts for completeness and accuracy that reflects services provided
Evidence-based practices
Current patient status
Individualized care plans
Progress toward goals and outcomes of care
Provides coaching and findings to the interdisciplinary team members as needed
Qualification
Required
Registered Nurse, currently licensed in Massachusetts in good standing
Associate's degree in nursing required; Bachelor's degree in Nursing preferred
Minimum three years of varied clinical experience as a professional nurse, and preferably one of the three years in a hospice, home care, or long term care setting
Understanding of hospice philosophy and principles
Experienced with Electronic Medical Records
Proficiency in MS Office/Excel/PowerPoint/Outlook
Able to learn and use healthcare specific data entry systems
Ability to interpret regulatory standards for documentation according to industry standards
Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation
Preferred
Bachelor's degree in Nursing preferred
Preferably one of the three years in a hospice, home care, or long term care setting
