Clinical Director PACE - Life Program - Full time - Bordentown

Responsibilities
Manages and oversees nursing and clinical support staff (i.e. clinic RNs, Home Care RNs, MAs Schedulers, Med Room RN, Medical Records/HIM) including coaching, leading, managing, clinical education, training/ observations, reporting requirements, and competency evaluation
Coordinates and supervises daily administrative needs of clinical support team; including care plan organization, training, direction, and collaboration. Provides clinical support decision making and direction as needed
Conducts audits, works with regulatory agencies and Quality Manager to gain insight on trends
Maintains regular attendance at, and participates in, all interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery
Supports nurse and clinical support employees by providing coaching and mentoring, ensure that clinic staff works within their scope of practice, implements team building, and provides feedback and redirection as needed
Ensures that job responsibilities, authorities, and accountability of direct reports are defined and understood. Performance evaluations of clinical team, corrective action oversight
Communicates effectively with Center Director and Medical Director to keep them abreast of any issues or concerns and to assure consistency with practices across the organization
Evaluates overall effectiveness of the clinic, implementing change and quality improvement as needed. Delegates process improvement and other tasks as needed
Ensures effective and efficient flow of the clinic; monitors needs and utilization of clinic appointments. Assists PACE team triage and determine acute needs as necessary
Acts as designee to communicate with IDT and community agencies as needed to support efforts of clinical team
Oversees and supports nursing and clinical support staff in ensuring signing of orders, ordering medications from pharmacy, and disseminating orders and instructions to appropriate personnel
Supports clinical team as needed by coordinating participant care with outside contracted service providers – nursing homes, assisted living, lab, oxygen, etc
Communicates with weekend and after hour staff, following up appropriately; Maintains clinical nurse on call coverage calendar
Assumes role of clinic nurse and provide direct patient care and management when needed to meet clinical operation's needs. Performs role of Clinic RN if needed. May need to assist team in providing assessments off site (facilities/home) depending on staffing needs and participant needs
Leads clinical team in identifying emergency or advanced care situations; collaborates with Center Director to set expectations for clinical team and ensures all decisions support participant needs and provides adequate care management. Assigns duties or tasks as needed. Adjusts clinic workflows based on acuity needs, census needs, and emergency/acute situations
Partners with Center Director to make fiscally responsible decisions for supplies and equipment purchases. Partners with Executive Director to forecast staffing needs related to annual budget
Participates in continuing education classes and any required staff and training meetings
Acts within the scope of their authority to practice
Qualification
Required
Bachelor's degree in Nursing required
Five years of clinical long-term care experience, including one year with frail elderly population
Five years of supervisory experience
AHA BLS
Drivers License
Registered Nurse – NJ or Registered Nurse – NLC multi-state
Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and public
Able to be congenial with other employees, customers and vendors at all times
Experience working with clinical charts and records
Proficient computer skills including Microsoft office programs
Excel in organizational and supervisory skills, with an ability to effectively handle difficult situations
Ability to develop strong interpersonal relationships with coworkers and participants
