The County of San Mateo is seeking an experienced Senior Community Worker for the Healthy Families San Mateo County Program. This role focuses on providing intensive home visiting services to support pregnant individuals and new parents, promoting health and improving birth outcomes in the community.

Responsibilities

Assess and monitor client needs and develop individual care/recovery/support plan based on the needs of the client
Prepare and/or coordinate the preparation of informational and outreach materials, basic correspondence and reports, and other documents regarding program activities
Assist staff in developing strategy for conducting and maintaining effective relationships with the community; interpret community needs and cultural patterns to professional staff members
Performs crisis intervention by arranging for special services to assist clients in addressing serious personal, financial, employment, and/or family issues; performs crisis intervention
Establish and maintain collaborative relationships with community resources
Conduct home and field visits to assess and monitor client needs and ensure adherence to individual care plans
Provide information and refer clients to appropriate social services agencies; research community resources for clients; work closely with other County staff to coordinate various functions of outreach and referrals
Provide educational services to the clientele served; schedule, arrange, and assist in the facilitation of educational programs and activities to support client health/wellness, recovery, self-sufficiency, or employment goals
Perform outreach to provide education and information to individuals who may be at risk and unaware of services and/or prevention methods; inform clients and community members of programs and services and promote participation of groups and individuals in program activities
Conduct outreach activities to community organizations to provide information about the program and resources and to secure health, welfare, educational, and other social service supportive programs on behalf of clients; promote participation of groups, businesses, and individuals in program activities; and coordinate the delivery of support services to clients
Identify home, family, or community conditions needing improvement and develop, recommend, and implement measures to correct these conditions
Assist in the follow up and monitoring of cases for assigned client groups; work with client and case management team to develop and maintain individual development plans
Monitor action plans and periodically make contact with clients to assess progress and identify support needed to aid clients in pursuing interim goals as planned
Advocate with external agencies, employers, and other organizations, to help ensure that the client is represented and informed of processes and procedures, to resolve issues and disputes with third parties, as well as engage in communications on behalf of the client
Assist clients in a wide variety of daily living activities, such as completing applications and forms, providing transportation, budgeting, and navigating support service systems
Represent the division or section on internal and community committees and task forces as assigned representing the needs of the community or of individual clients; attend and participate in community events
Act as an interpreter in contacts involving non-English speaking residents
Attend staff and in-service meetings
Develop immediate solutions to emergency problems and expedite delivery of needed services
Perform special assignments, projects, or other related duties as required

Required

Fluency in Spanish is required
Two years of experience providing community services of a remedial and educational nature which includes experience counseling and conducting outreach to the client population served
Possession of a class C California driver license or equivalent
Experience in conducting home visits with clients and assessing problems and needs
Experience in working with pregnant and parenting women
The ability to serve as a trusted liaison to perinatal providers with differing practice styles and environments
Background on normal child growth and development and parent-child relationships
The ability to relate to families from a strength-based model, even in an apparently chaotic family environment
The ability to recognize child abuse and neglect
The ability to work independently as well as within a multi-disciplinary team
Knowledge of trauma-informed care and adverse childhood experiences
The ability to problem-solve, make independent judgments, and work autonomously
Communicate effectively both orally and in writing
Be flexible adapting to a changing environment
