RN Home Based Primary Care
The Marion VA Healthcare System is seeking a full-time Registered Nurse to serve in the role as Case Manager for the Home Based Primary Care Team, located in Vincennes, IN Community Based Outpatient Clinic. The Registered Nurse in this position provides health care services and care management to high risk patients in their homes.

Responsibilities
Case Management
Patient Assessments
Participation in interdisciplinary team process
Home care visits
Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care)
Completes and submits clinical notes
Observes patient for changes in conditions and reports appropriately
Provides education instructions to patients and their designated significant other
This position requires that the employee operate a government motor vehicle
Performs other professional nursing duties as assigned and within their scope of practice
Other duties as assigned
Qualification
Required
U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment
Selective Service Registration is required for males born after 12/31/1959
Subject to background/security investigation
Selected applicants will be required to complete an online onboarding process
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)

English Language Proficiency
Graduate of a school of professional nursing approved by the appropriate accrediting agency
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
Preferred
Three (3) years nursing experience with some hospice or palliative care experiences and BSN preferred
