Nurse Practitioner, Internal Medicine- Geriatrics

The University of Texas Medical Branch is seeking a full-time Advanced Practice Provider within the Division of Geriatrics-Home-Based Primary Care Program. This role focuses on delivering patient-centered primary care services to medically complex, homebound older adults, aiming to enhance their quality of life and reduce hospitalizations.

Responsibilities

Provide comprehensive primary care services to geriatric patients in home and nursing facility settings, focusing on prevention, chronic disease management, and functional well-being
Performs individualized history and physical examinations for diverse populations inclusive of social determinants of health
Orders and interprets diagnostic testing in accordance with relevant health concerns of the patient
Obtain laboratory testing-includes blood specimen draws, catheterization, flu swabs, and joint injections
Precepting and mentoring school of nursing and school of medicine students
Develops a comprehensive plan of care that encompasses evidence-based practice guidelines, practice protocols, Medicare, Medicaid, and other state and federal regulations
Prescribes appropriate pharmacological and non-pharmacological treatments
Provides appropriate education for current health diagnoses as well as anticipatory guidance for health promotion and disease prevention applicable to the individual’s health and socioeconomic status
Monitors plan of care for cost-effectiveness and the achievement of expected improvements, while openly seeking feedback from patients, significant others, colleagues, allied health, appropriate referrals, and collaborating physician to enhance quality outcomes
Works independently to full scope of practice in compliance with the Texas Nurse Practice Act with acknowledgement of collaborating physician availability for consultation and collaboration
Collaborate with interdisciplinary teams—including nursing, pharmacy, social work, and rehabilitation—for optimal care of older adults
Contribute to curriculum development, clinical skills training, and geriatric-focused academic programs

Required

Completion of a graduate level (or higher) nurse practitioner program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
No experience required
Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
Licensed Registered Nurse (RN) by the Texas Board of Nursing (TBON)
Licensed Advanced Practice Registered Nurse (APRN) in a nurse practitioner clinical area of specialization by the Texas Board of Nursing (TBON)
Master's or Doctoral degree in Nursing or Physician Assistant Studies from an accredited program
Current Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA-C) license in the State of Texas
National certification in Adult-Gerontology Primary Care, Family Practice, or related specialty

Preferred

Experience providing primary care to geriatric or medically complex populations in home or long-term care settings
Experience in an academic medical center or teaching environment
Strong interpersonal and communication skills with a collaborative, team-oriented approach
