Responsibilities
Serve as part of a multidisciplinary “Care-Team” for members with Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD)
Participate in Multi-Disciplinary Team (MDT) meetings and contribute behavioral health-informed insights
Join Lead Care Managers for in-person community visits to support member engagement and service coordination
Assist with care planning, documentation, and linkage to resources and services
Promote integrated care by collaborating with internal teams and external partners
Performs other duties as assigned
Qualification
Required
Active California license as an LCSW, LMFT, LPCC
Experience working with individuals with high behavioral health needs, including SMI and SUD
Knowledge of Medi-Cal systems and Enhanced Care Management is strongly preferred
Excellent communication and collaboration skills
Ability to work effectively in both community-based and office environments
