University of Florida Jacksonville Physicians, Inc. is seeking an Advanced Practice Provider I (APP I) to provide direct patient care in the areas of Primary Outpatient Care and General Pediatrics. The role involves obtaining patient histories, performing examinations, formulating health management plans, and educating patients about their health care.

Responsibilities
Obtains patient histories, performs physical examinations and/or diagnostic and therapeutic procedures, as well as orders, interprets, and evaluates diagnostic tests
Formulates comprehensive health management plans for patients in collaboration with physicians, pharmacists, and other medical professionals to include progress notes, narrative discharge summaries, consults, etc. Facilitates patient participation in making health care decisions
Reviews, prescribes and/or recommends medications and treatments to patients
Provides timely and appropriate referrals to patients in need of further medical care and services
Educates and counsels patients/families regarding their health care, including routine health maintenance
Enters pertinent clinical information in the patient’s medical record in a timely and accurate manner and in accordance with established compliance billing guidelines. Documents and closes open encounters, MyChart messages, and other EMR communications in accordance with institutional requirements
May provide education and instruction
Call coverage may be required specific to departmental needs
Qualification
Required
2 years Outpatient setting
High School Diploma or GED
Masters Nursing
Masters Physician Assistant program
Advanced Practice Nurse Practitioner License
Physician Assistant License
Basic Life Support (BLS)
Preferred
Advanced Cardiovascular Life Support (ACLS)
PALS Certified
