Responsibilities
Support daily team operations and care coordination
Provide clinical supervision and direction to team members
Conduct side-by-side, field-based visits with staff and recipients
Participate in treatment planning and team meetings
Deliver direct services to recipients as part of the clinical team
Guide staff in implementing and documenting Individual Action Plans (IAPs)
Oversee training in life skills, community integration, and self-advocacy
Communicate with families and external providers as needed
Participate in on-call rotation and complete special projects
Qualification
Required
Master's in Social Work or Mental Health Counseling with current NYS LCSW, LMSW or LMHC license
Minimum two (2) years of experience in behavioral health, preferably with mobile/multidisciplinary teams
Valid NYS Driver's License and clean driving record
Ability to travel daily throughout Nassau County
Excellent communication, time-management, and organizational skills
Proficiency in Microsoft Office and electronic health records (EHRs)
