Track and analyze scholar participation and learning data to inform planning, differentiate instruction, and ensure alignment with program expectations and milestones
Collaborate with colleagues and instructional leaders to continuously improve curriculum implementation through reflective practice, peer feedback, and shared planning
Co-create and maintain a holistic, multi-phase Consolidated Education Plan (CEP) for each scholar that supports their growth from high school to college and into the College Thrive phase
Support scholars in understanding their academic strengths and needs, and use this insight to guide individualized advising and success planning
Facilitate scholar self-exploration of their social-emotional development, mental health needs, and early college and career interests, using the CEP as a central tool for reflection, tracking, and planning
Leverage partners and external Community Based Organizations (CBOs) to align supports and ensure wraparound services for each scholar as defined by CEP
Work closely with College Counselors and College Advisors and College Thrive Coaches to deliver a seamless academic advising experience across all scholar phases and ensure alignment in postsecondary preparation
Lead proactive outreach and coordination with school counselors, teachers, and caregivers to build a unified support system around each scholar
Maintain a caseload of scholars, ensuring each scholar is seen, known, and supported across their academic and personal development journey
Track academic performance indicators (e.g., GPA, coursework, graduation requirements), interpret data to drive scholar-facing conversations, and implement timely interventions as needed
Use scholar data routinely to inform intellectual preparation, instructional delivery, and individualized advising. This includes weekly and monthly review of both leading and lagging indicators
Apply data insights to prioritize supports, adapt curriculum, and co-develop action plans in collaboration with college counselors and advisors
